Mom's Quit Connection for Families is a free smoking cessation program available to pregnant and parenting individuals, their partners and family members who live with or care for children 7 years old and younger. Given the influential role of health care providers in cessation efforts, the program offers a package of specialized services to assist providers in helping patients quit tobacco. Services include customized on-site staff training, webinars, streamlined referral process to link patients to free quit programs, professional development and patient educational materials.
Providers who refer patients to Mom's Quit Connection for Families can feel confident in that decision. The program employs a highly effective cessation model with quit and reduction rates that consistently exceed national averages. Patients are coached by a Certified Tobacco Treatment Specialist trained by Rutgers Tobacco Dependence Program. Coaches work one-on-one with patients to develop a personalized quit plan and continue that relationship with ongoing support, education and guidance. Providers who refer patients to Mom's Quit Connection for Families receive a status report on those referred.
