Mom's Quit Connection for Families

Mom's Quit Connection (MQC) for Families is a free smoking cessation program available to pregnant and parenting women, their partners and family members who live with or care for children 7 years old and younger. Given the influential role of health care providers in cessation efforts, MQC for Families offers a package of specialized services to assist providers in helping patients end a tobacco addiction. Services include customized on-site staff training, webinars, streamlined referral process to link patients to free cessation programs, professional development and patient educational materials.

Providers who refer patients to MQC for Families can feel confident in that decision. The program employs a highly effective cessation model with quit and reduction rates that consistently exceed national averages. Patients are coached by a Certified Tobacco Treatment Specialist trained by Rutgers Tobacco Dependence Program. Coaches work
one-on-one with patients to develop a personalized quit plan and continue that relationship with ongoing support, education and guidance. Providers who refer patients to MQC for Families receive a status report on those referred.

To begin referring patients, complete the form below and fax to 856-665-7711. An MQC for Families team member will be in contact.

Ask, Advise, Refer: Brief Intervention Model

Ask, Advise, Refer (AAR) is the CDC’s evidence-based brief intervention model. This free training will assist providers and professionals in quickly and effectively implementing a dialogue about tobacco use among their patients. Providers will gain information on the integration of the model into existing office routine, patient resources and fax-to-quit referral forms for MQC for Families and the New Jersey Quitline. MQC for Families offers both on-site and online training to those interested in receiving AAR training.

Additional Tobacco Cessation Training
