Mom's Quit Connection is a free smoking cessation program available to pregnant women and new moms. Program staff work closely with and provide specialized services to health care providers treating patients addicted to tobacco. Services include customized on-site training, webinars, streamlined referral process for free cessation programs, professional development and patient educational materials just to name a few.
Providers who refer patients to MQC can feel confident in that decision. MQC implements a highly effective cessation model with quit and reduction rates that consistently exceed national averages. Specially-trained MQC Quit Coaches work one-on-one with women to develop a personalized quit plan and will continue the relationship, providing ongoing support, education and guidance necessary for success. MQC also sends providers regular status reports on patients they have referred.

Ask, Advise and Refer: A Brief Intervention Model
Ask, Advise and Refer is the CDC evidence-based intervention model designed to assist providers in quickly and effectively implementing a dialogue about tobacco use among their patients. MQC offers training both on-site and online to those interested in implementing the protocol in their practice or facility.
Creating a Smoke-Free Home

To refer to the NJ Quitline, complete the form below and fax to 1-800-483-3114. A Quitline counselor will contact those referred to answer questions and enroll those interested.
