Case Management

Case managers work one-on-one with individual clients. They help clients develop individualized care plans that outline the health care and social service needs of the woman and her family and then provide assistance in addressing those needs. Women enroll in case management for two years.

Fathers on Track

Fathers on Track is a peer support and education effort in which fathers help other fathers become positive role models, partners and Dads. Subjects tackled include money, family, parenting, careers, anger management, and relationships. The free program is open to any father and father-to-be who lives in Camden and wants to be a better parent. Program services include: individual and group coaching and education; help in developing coping skills, vocational/employment training or GED referral.

Una Nueva Esperanza

Una Nueva Esperanza was developed specifically to address the needs of Camden’s rapidly growing population of undocumented pregnant women. It offers hope for those who find themselves isolated and ill-prepared to navigate an unfamiliar health care system. A support group fulfills the need for community and education and case management services help improve birth outcomes. Culturally appropriate workshops educate clients about health behaviors, prenatal care parenting skills, child development and home safety.
